Download the affidavit form to be used instead of carrying birth certificates for your players. One person from each youth association should be in charge of verifying all birth certificates for ALL teams that are going to play teams from other associations. This is a required form. Please fill out the form as needed, make sure your association's designated person verifies each birth certificate and have the form notarized.
After doing so please send a copy to me so that IAHA has a record. We need the forms submitted before the end of the year.
